BankUnited offers two dental plans, a DHMO and a PPO plan, to eligible employees through MetLife. All plans provide the same full coverage for preventive care services. To enroll in any of these plans during an eligible timeframe (i.e. Open Enrollment, New Hire Enrollment, or Qualifying Life Event) log in to  ADP Employee Self Service Portal.

Things to know:

  • Only Available in Florida and New York.
  • All (non-preventive) services have a fixed copay as listed in the fee schedule (price list).
  • There is no deductible.
  • Participants must designate a primary care dentist that will manage overall care.
  • Dental providers that participate in the DHMO network are limited.
  • No out of network coverage available.

  • PPO Dental ID Card– Metlife does not issue ID cards for the PPO plan. Your SSN is your ID number. The BankUnited group number is 150917. You may use this generic ID card but it is not required.
